Regarding the power/ground pin table, if you use implicit power/ground pins and
POWER_GROUP properties in Cadence/Concept, the power/ground pins may NOT be
shown on the component schematic entities, so a table is most helpful. If
you're using ORCAD as your schematic capture vehicle, you end up showing the
power/ground pins explicitly and the table may be superfluous.
Putting the cross-reference info on the first page of the schematic
standardizes where you need to look to find it. If it's a 10-page schematic
and you don't routinely put the cross-ref material on the first page, you have
to flip through multiple pages to find it. Better to have it in a consistent
spot whether the schematic is 3 pages or 30.

I see no reason for that one these days. What's your reason? All
power/ground pins should be shown on the components, and large devices have
them on separate symbols, as we've discussed here.

I'm not sure I'd put that on the first page, but it isn't a bad idea to put
that information in a table somewhere.
