You hit it right on the head!
My page.map file looked like this:
8 1
6 4
9 3
13 2

This was because the E.E. cloned pages from prior designs (pages 8,6,9 &13)
Named the .csa's to 1-4.
This cleared out the page.map file since numbering was correct. (zero K file size)

1. I changed the "old" page numbers in the .csa's to match the .csa.
2. Repackaged to Allegro.
3. Quick place by page then worked fine.

the page.map file shows the order you really want them in.
So in the case of place by page you might seem the page number 5
on your schematic and try to place by page all components on page 5.

Well, no, I haven't had the problem, but that's because
I haven't used the "place by page" feature. Is that
cheating? In any case, I recommend contacting Cadence as
you haven't gotten any replies from the group. Usually
a sign that no one has an answer. If you do find a
solution, please let all of us know.

As I mentioned, it does work "sometimes" and if anyone else uses this feature, (which is very useful when it works), have you run into this inconsistency? It is very easy to setup... select "place by page" and type in the page number to place.

I take it you have not run into this problem.

It really seems to be a "hit or miss" feature that is GREAT when it works.
In many designs it will not work at all.
