You and your colleagues are cordially invited to attend strategic planning
town hall meetings hosted by the Hawaii State Department of Health, Chronic
Disease Management and Control Branch (CDMCB), Healthy Hawaii Initiative
(HHI).  Involvement from key partners, leaders and stakeholders in Hawaii's
communities, schools, worksites, health systems and state agencies is
critical build a cross cutting plan that will assist all partners work
together on shared objectives.  We all must work together to reduce exposure
to tobacco products and second hand smoke, make it easier to access healthy
foods and be physically active, and make sure people can easily manage their
chronic diseases.

 

The Chronic Disease Management and Control Branch (CDMCB) is pleased to be
partnering with the Healthy Hawaii Initiative (HHI) to broaden the scope and
reach of the coordinated chronic disease strategic planning effort by
incorporating primary prevention (physical activity and nutrition) in the
planning process. This will strengthen the comprehensiveness and capacity to
address chronic disease from both a primary and secondary prevention
perspective.

 

The Coordinated Chronic Disease Plan will develop a framework for
communities, organizations, state agencies, advocates and leaders to see
where and how we all fit and contribute to the larger vision of a healthy
Hawaii.  The plan will establish and prioritize overarching policy,
communications, epidemiology, surveillance and evaluation activities to
support an array of categorical chronic disease programs and primary
prevention strategies.  

 

The Coordinated Chronic Disease Plan will act an umbrella plan for all
chronic disease and primary prevention plans.  Each of the categorical
chronic disease programs and primary prevention areas will continue to
update and develop plans specific to their areas.  These categorical plans
will contain additional detailed strategies, tactics, and measurable
outcomes going beyond the big picture framework the Coordinate Chronic
Disease Plan will provide.

 

The purpose of these meetings is to: (1) share current highlights and
successes; (2) provide data on the burden of chronic disease and social
determinants of health; (3) gather statewide community input to begin the
process of developing a five-year coordinated chronic disease strategic
plan; and (4) provide informal networking and sharing opportunities.
