Issue
...Part of the issue was that they entered three Housing placement records for
one client, when there should only be one. It went like this: someone entered
the housing placement record, two days later someone else entered it again,
this time with a little more detail - they should have simply edited the
existing one. Then three months later, when they went to do a follow up, they
created a third placement record with the same information and then recorded a
FUP along with it.
