Looks very nice.
I'm wondering if the options under Audio Mixer | Setup are really 
necessary. Probably I'm not an audio nut enough... Anyway, if so, the 
nested tab view could be avoided and you'd just have the gain controls 
there.
What could be added some time are VU-meters instead of the current 
slider bars. Make them as wide as the slider knobs. Would make some 
nice eye-candy as well. :)

WRT the Preferred In/Output checkboxes in the Audio Settings tab: How 
about adding two columns to the listview, "In" and "Out" and radio 
buttons for each entry?

I also noticed that there are still some strings not conforming to our 
system wide casing, e.g. "Audio Settings", "Preferred Input", "Output 
Frequency" etc.
