Note that this isn't/hasn't been just one bug. The assertion is triggered
on an inconsistency in the page tables, which can have different causes. I
fixed one five months ago, with at least another one remaining, and
apparently the scheduler branch merge introduced a new one. Unfortunately
we can't discriminate the causes just from the assertion message.
