We have the parent sign an acknowledgment form stating that they have received 
a copy of the Parent & Student Information Guide.   Our AUP (included in the 
handbook).  The acknowledgment form states that if parents do not want a 
student to have access to the Internet, the parent  must inform the principal 
in writing within 10 days of enrollment or within the first 10 days of the 
school year.  Parents must initial this statement.

This has saved a lot of work on our staff because we code all students as Yes 
for internet use at the beginning of the year and only change the ones that 
parents send in a request to deny.  If you would like a copy, let me know.

We do the same thing.  What I am trying to decide is, if we send home a student 
demographic sheet twice a year and on that sheet we ask if the student can have 
Internet Access and refer to the AUP in our Code of Conduct in the handbook. 
Would this work as far as the documentation we need on file. Then we are only 
keeping up with one sheet of paper instead of two.
