Have not seen that one yet.......but I'm not surprised.   I'm having a
whale of a time trying to get cooperation and phone call answering on SR
errors with Cyber concerning overlapping enrollments.   This seems to
happen every year.  Me thinks they feel entitlement to their own way of
doing things.

for records, they want this form notarized and a copy of the parent’s id
sent with the form.
