Neville, I will find the medical detail soon..  Meanwhile where in the bible 
does it say rope was used. or that it was in the palm... I ask this despite Mel 
Gibsons depictation. 

Was it not an unusual method of the time  that nails were used?

  Just one, small comment. You said that, "the nails had to be further up near 
the wrist." That is not true, because the forearm would be bound to the beam 
with rope at the wrist location. The nail can then be positioned exactly where 
the Bible says. Indeed, there are far too many small bones in the wrist for 
several of them not to have been shattered by the nail.
