This can happen if you are loading any version software below 6.4....
As stated in document B0400RU .....Page 48.....The specific doc that ships
with the GX240 station.
After loading I/A software you must select NO to reboot and perform a
diskette task.....see excerpt from document below:

B0400RU - Rev A 2. Installing I/A Series Software
Restarting Your System
When the I/A Series software installation is complete, the setup program
asks you to reboot your
workstation as shown in Figure 2-12. Do not reboot the workstation with the
new software, select
NO, I do not want to restart my computer now in the display window and click
on Finish.
Loading the I/A Series Driver for the GX240 P93
You must perform the following procedure to allow the workstation to boot
after installing I/A.
The driver updates some I/A files required for the GX240 P93 and the
multi-port serial card to
work correctly with the I/A Series system. If the following procedure is not
performed
for I/A Series Software Version 6.3 or earlier, the system will not boot.
To load the I/A Series driver for the Multi-port Serial card and other
required files:
1. Insert the floppy disk K0175VY
2. In the Start menu, click on Run.
3. Enter: A:\setup. The files are copied.
4. If no Equinox driver you will get an error.
5. Re-boot the system.
NOTE
If the above procedure is not performed for I/A Series Version 6.3 software
or earlier
the system will not boot
