If all these parameters look fine then I had a similar problem on a B Box
one time.  I had to enter the following command to get it to work properly.

When an HP895Cxi (or other PCL3) printer is attached to its
parallel port,  it also needs the following command entered in a VT100
window:
   lpadmin -p LPxx -H,  where LPxx is the logical name of the printer on the
parallel port.

Doing this once should clear up the problem,   Hope it works for you as
well.
