I worked at a site that used scripts to change environments (or perhaps there was a dm command in the environment file, I can't remember). But I do remember that it wrote the environment name and date-time to a text file every time it was run. There would be a separate environment (and password) for each operator, engineer and tech. This worked fine for the individual operators who didn't have access to root, but someone with root-access could easily have modifed the text file to cover his/her trail.
