Process Hazards Assessment were routinely performed at The Foxboro Company,
when they had a Reliability Engineer.  I was the reliability engineer in USA
(FOXMASS) and Lubbo Winkel carried the load in Europe. Now we have both been
forced out of Foxboro, and there isn't any reliability effort left in the
dear old company, whatever it is called.

Dear Terry, I consult on the side and would be willing to assist you.
However, if that would be too dear for you, kindly refer to Dr. William
Goble's book,
"Control Systems Safety Evaluation and Reliability", ISA Press.  ISBN
1-55617-636-8.
I copied the ISBN directly from the book cover. This is a good "cook book"
with instructions on how to do such assessments and much more (e.g. Markov
System Analysis).

If you have access to Amazon.com, read my review of his book.  I gave it a
five-star rating and that is my opinion as listed in the public domain.

PHA is a time-consuming but worthwhile endeavor, rarely attempted and even
more rarely supported by management.
