Yes, both of the red and green LED's are illuminated on the failed CP.  Looking 
at the alarm printout, here is what I see:
 
Single PIO Bus Access Error on B
Single PIO Bus Access Recovery on B
FCM B module failed
FCM B no longer failed
 
These have came in multiple times.
