We have this type of failures regularly, but on FBM41/42, never had it
on FBM04. We had to change the mounting structure to get the problem
solved in some cases. Usually swapping cards and or cones solves the
problem. We also had some problems with letterbugs, pushing the
letterbug firmly into the fitting solved this problem. Never had to
change the letterbug though. I would expect faulty wiring to trip fuses,
not cause a card to go offline. You might also want to check if the
eeprom update of the failing card is still up to date and the same as
the other cards.

Hi All,
I am having a problem with one FBM04. Last month it went red/green
and
failed, but as I was removing the nosecone it rebooted and went back
online.
I pulled the FBM and reseated it in the backplane a few times, & did the
same with the nosecone and the letterbug in case they had dirty
connections.
The FBM came on line normally and worked fine.
The sysmon log had three messages about the failure:
05/01/06 06:00:52 sysmonmsg
-00057 Currently using PIO bus B=20
-00079 PIO Bus Access Error on A & B=20
-00043 Equipment failed=20
Eight days later the same FBM04 went off line. Sysmon had these
messages:
05/09/06 17:55:45 sysmonmsg
-00079 PIO Bus Access Error on A & B=20
-00056 Currently using PIO bus A=20
-00043 Equipment failed=20
This time I installed a new FBM04 and put the old FBM04 in a spare
slot in
another node.
Twenty days later the FBM04 went offline with the same three sysmon
messages, so I replaced the nosecone with a new one, and put the old
nosecone on the original FBM in the other node.
Yesterday the FBM04 went offline again, and again it had the same
sysmon
messages. I pulled it out of it's slot and installed it in a different
backplane (same letterbug ID & host CP). If it fails again the only
thing
left to replace is the letterbug. We had a few letterbugs with short
pins
many years ago, but this one looks fine and doesn't appear damaged.
Can the field wiring for the four inputs and four outputs cause PIO
errors?
In between failures the FBM has been working properly, but it is
getting
annoying having it fail for no apparent (to me anyway) reason.
