Hi all,
We have had the same problem on a dual headed WP51 style D. The problem was the
video card. Some of the video cards doesn't work with I/A V6.4 software.
This issue is, as far as we know, not documented.

Hi list:
We are upgrading a WP51D from 6.1.2 to 6.4 with a dual monitor
using
a PGX32 (Raptor GFX OpenWindows for Solaris) video card. We followed =
the
standard procedure included in BO400PT but it didn=B4t work. During =
the
booting process the second video card isn=B4t detected.
Any idea?
