[foxboro] : Chemical Safety Board Video

The USA Chemical Safety Board has released a 56 minute video that
explains the causes, the root causes, the refinery management errors and
the errors in BP Corporate policy that combined to result in an
explosion in 2005 that killed many refinery workers.

 

Since it involves instrumentation and DCS as well as graphic displays,
it is a worthwhile watch.
