One other thing to look at is how long the interval was between re-enabling and logging on. If you tried immediately, it could have failed for that reason. One other possible reason is the msexchangeuseraccountcontrol setting. See http://support.microsoft.com/default.aspx?scid=kb;en-us;296479 for more information on that. In the earlier days of Exchange 2000 it was possible that if you disabled the mailbox-enabled user object, it set the bits on this attribute causing another attribute to also be set. This didn't get unset if you later renabled the object. I've heard that this behavior has since changed, but haven't seen it to confirm. Easiest way to fix that is to remove the exchange attributes via ESM wizard (note: copy the addresses first since they'll go away), let replicate throughout the Active Directory, create a new mail store for the user, disconnect the new mailstore, and re-connect the user to the existing mail store. You may want to export the data first if you're not comfortable with this process or are not using deleted mailbox retention. I know if you delete a user and recreate, you need to go into the exchange manager and reconnect his mailbox to his account.
