Simplest way is to NAT the public IP to the Private IP across your firewall. So for example 212.23.45.121 on the public side of the firewall translates to 192.168.1.2 on the internal side Depending what services you are running on the Exchange box you could also consider only exposing port 25. So only traffic to port 25 is routed to the Internal server.
