User Cannot Open Any Custom Forms

One user started having trouble with one specific form last week. She cannot
open it, sometimes she gets message 'Custom Form could not be opened. Outlook
will use an Outlook Form instead." and other times she gets simply "This form
cannot be displayed." We have cleared cache, restarted Outlook, run detect &
repair and rebooted the system. Now, this morning the problem is worse: she
gets the 'Custom Form could not be opened' message when she tries to open ANY
form. HELP!
