Send As permissions

Under Active Directory, User A has Send As permissions for User B. 

User B has also shared his calendar to User A by giving User A
Publishing Author permissions. 

Using Outlook 2003, User A can create new appointments for User B, but
the notifications that are sent out are something like 'User A on behalf
of User B...'

How can I set this up so that the appointment notifications are sent as
User B?
