If you have more than the default 32 mapi sessions; you will not be able to
connect with your Outlook using MAPI. Having multiple mapi sessions could be
caused by third party applications; network issues etc that are not properly
closing out sessions.
You need to enable diagnostic logging to see if you get the 9646 event id.
Event ID 9646 is logged in the application event log of your Exchange Server
2003 computer when a client opens many MAPI sessions

Could be serveral reasons; 1 you could be maxing out your 32 default
sesson limit. Turn on diagnostic logging for Exchange IS Mailbox logons.
