I don't know how much it costs, but you might want to look into I Hate Spam
from Sunbelt.
If what you want is something that runs on Exchange directly.

If not, I'd run SpamAssassin with Amavis-new on a linux box with Postfix.
Thats what i run here and i've been very happy with it.
All you need to buy then is the hardware and support from Redhat if you want
that.

The docs are pretty good and the respective mailling list even better
