Please Help IM problem

please help
i think have tried just about every possible senario i can think of, i am
setting up IM on my exchange and i have no problems in house with the
setup, i have everything working correctly
but when connecting from the outside i am not able to connect
i have port 80, 53, 389, all going through the router, but cant seem to
get authenticated in my opinion
i get the subscribe on the log file, but it returns with a 404 message
with the following 4.6.0.84
im lost for whats is wrong it is making it to the server, could it be a
return problem?
