ESE runs a check ever night as part of online maintenance. If you were
having database problems, your application event logs would be
displaying errors. If you want to run an offline consistency check, you
can do that by using eseutil; but based on the problem description, I
doubt that is the issue.
To further diagnose, I'd want to know what the bounce backs are, and
what happens when a remote (WAN) user tries to manually do a SMTP
session using the telnet tool...

My mistake, it was around the time we installed Windows SP1 which was
installed on Apr. 19, while ms05-19 was installed on Apr. 13. Exact time
problem began occurring was not recorded.

Since installing Exchange 2003 SP1, I have had to reboot the mail server
every morning. Inbound email stops and senders get bounce backs.
Internal messages continue as well as outbound. No messages are held in
queue, services are running... Service restarts do not fix the problem.
After reboot inbound continues again.
