No, one copy, it uses Single Instance Storage

Exchange Server 2003: If a user in the same storage group and DB sends a 5MB
attachment to five other users, are five copies stored in each persons
mailbox, or a virtual pointer is created and directed at one copy of the
attachment for all the recipients.

Just trying to wrap my head around the storage system.
