I have ports 80 and 25 forwarded from the ADSL router to the external network card on each server, aside from that all that I needed to do was create the "in our case" permanent back to back VPN connection. Once the connection is established and you can ping the remote server it should all work, I used the Outlook mail setup wizard, told it to use exchange for e-mail, entered the ip address of the exchange/SBS 2000 remote server, and then entered a user name which if all is well the system will verify the user immediately, and away you go. I hope this helps but I will add that this may not be the accepted way of achieving this aim, (for security and or operational reasons) if it is not then I would welcome feedback to me.
