OK.

Then don't even think about using ESM to do this.

In ISM, rightclick on the website and export to file. Edit the file
(it'll be pretty obvious if you've dealt with this stuff). Reimport as a
new website.

Practice in your test server first.

Ideally I want to be able to offer someone the option of using both OWA
FBA and RPC over HTTP at the same time. I am using ISA 2004 Server and
hope that http://www.isaserver.org/tutorials/2004pubowamobile.html is
exactly what I need to do on ISA's side.

I can answer the question, but before I do that - what is your goal
here?

Exchange virtual server.

When you create a EVS under 2003 it creates three child nodes in the
root of the EVS which are:

/exchange

/exchweb

/public

/exchweb is the only one visible out of the three. Therefore should I
bother creating /exchange /public /OMA /MS-active sync folders in the
root of the EVS?

RPC over HTTP

Is there anyway of adding the /rpc folder to the list of child nodes
under the EVS, or do I have to install it manually?
