Last year I used Surveyshare to conduct a survey and was very please
with their free service. Strongly recommended: fast, efficient,
well-designed user interface, downloadable results, lots of stats
functions and tweaks for a free tool. Certain restrictions ("nuisance
factors" intended to make you want to upgrade to a commercial version)
can be easily bypassed.
