We were also asked to fill out forms in order for kids to continue to get accomodations. The forms ask the teacher about how a pupil's difficulties are exhibited in class and we were also asked to test the pupils with and without their accomodations.

I see nothing wrong in involving the teachers in determining dispensions. After all, the teachers are the ones who both pinpoint and have to deal with learning disabilities.
