[etni] Feedback Forms

Dear ETNI colleagues,

We have been told that we must issue feedback forms to our Juniour High 
English students. This is part of a schoolwide campaign for feedback on our 
teaching, on classroom atmosphere and on student self-evaluation on their 
own progress and behaviour.

Every year, as English teachers, we've given out questionnaires in order to 
improve our teaching, but this year, the school requires a more general form 
to be used for all subjects.

Our stance is that evaluation is a profession in itself. If objective, 
accurate results are expected, then the right questions need to be asked and 
then information has to be properly compiled.

So, my question is this: Has any school hired professionals to create an 
objective feedback form?
Is there a teacher who has at her/his disposal a form that could be sent to 
us?

Meanwhile, we're using our own 2-page form, but find it sadly lacking. On 
the other hand, we are not professionals at evaluation and do not know how 
to properly create a new questionnaire.

If you can help, or make suggestions where we could turn, please let me 
know.
