Perhaps your configuration data is incorrect ?
What are the icons specified for the missing ones, in the pane1/bar, pane2/bar,
toolbar and commandbar pages of the configuration dialog ?
It could be that your displayed icons are all from gtk, while the missing ones
are all the custom ones ? In which case, it's probably looking in the wrong
place for the custom icons.
