I've been concerned mainly with files actually on a case-insensitive
filesystem, because once you get them off there, it's very easy to change the
case of each name and/or extension with the rename plugin.
I've added a configuration option as suggested, in case the "proper" approach
is too much fuss. I intend to try to get the kinks out of the automatic
detection.
