You can set up a "File Association" such that files with a
specific extension are always opened by a specific program.
However, this can cause you problems if now and again you do
not want to use that program.

If you're trying to open an attachment from within the body
of an email message, and you are a jaws user, you can hold
down the insert key on the numpad and type the letter a.
That will highlight the attachment, then you can just hit
enter.  I've never been able to get Duxbury to open an
attachment from an email message because it always wants me
to specify the program to open it.  I just do save
attachment from the file menu and save it on my hard drive
then open from within duxbury.
