I can answer some of it:-)  Yes, useing a fixed cheek bit will keep the bit
from rotating and pinching the lips of the horse.  If you want to use a bit
that has swivel cheeks but want to avoid the pinching you could use a
Butterfly type cheek piece rather than a liverpool type.

You can avoid some of the pinching by the way you buckle the coupleing rein.
Run the rein from the outside and through and around to front and back and
buckle.  (Would be the rough cheek setting)  Not sure if you can picture
it:-)

favorite bit and have it welded)
