I am now in the middle of implementing this and came across one problem:
Apostrophes are interpreted as "singlequoteclosing" (have always been in DokuWiki when $conf['typography'] is turned on). Although this is already semantically incorrect, no one ever complained, because the English "singlequoteclosing" (&rsquo;) looks almost like an apostrophe. But when quotation marks in other languages come into play, the result concerning apostrophes is definitely undesirable! 

Example outputs:
«He thought “It”s a man”s world”...» (ca)
»He thought ›It‹s a man‹s world‹...« (da)
«He thought ‹It›s a man›s world›...» (el, es, et)
« He thought ‹ It ›s a man ›s world ›... » (fr)
(And so on ... I only just reached the letter "g" ... ;-) )
Do you mean the source text is %%"He thought 'It's a man's world'..."%%? (I use %% as string delimiter to try to avoid confusion with the in-string quotes... I hope this does not create _more_ confusion...)
I would find it quite disturbing to enter %%"%% and get %%« %%. (Note : I choose the French example because it is my mother language.) I believe French users most of the time write " when they want to get “/” and use << to get «. Otherwise they quite happily use English-style quotation, even for official publications and in newspapers. (At work, most documents produced use English-style quotation, and I work for an administration that is responsible for French culture... I am the only nitpicker that insists to use French-style quotation.)
As a side note: while the secondary quote level ‹/› is technically correct, I never have seen it used outside of typography manuals. Most people are unaware of the primary / secondary quote levels distinction and those few who are most often use «/» at first level and “/” at the second level (sometimes the inverse is prefered).
OTOH, MS Word and OOo Writer _do_ automatically substitute %%« %% / %% »%% for %%"%%, but it happens in a WYSIWYG way.
I still believe that, in accordance to the least confusion principle, Dokuwiki should not substitute «-style quotes for ".
