You added a "." in front of the pdf-file.
If you use my example, it works (for me):
[[ :path:to:file.pdf |{{path:to:image.png?200|}}]]
Don't use the point.
Maybe I don't get your problem right. Could you explain it again and
with more details?

above example shows image, that is linked to (non-existent) page:
pdf:file.pdf.txt
That is the whole problem. How do I tell dokuwiki that I want to link to
file.pdf, instead of file.pdf.txt ?
