Krzysztof, I've been in the unfortunate position of having a Windows server with IIS mandated to me. In this configuration I've been able to achieve the same thing by setting the IIS virtual directory (or whatever IIS calls these things) to force integrated security and setting windows permissions on the files to give everyone read access. This has worked very nicely for me as IIS seems to set the full username i.e. DOMAIN\username as either AUTH_USER or REMOTE_USER (not sure which one) that DokuWiki seems to pick up nicely. However I love the patch you have sent in, I just need to talk people round here into letting me have a Linux box on site. Good luck.
