[dokuwiki] Registration Page - Issue

Hello,

I just started using DokuWiki and I like it a lot. Recently there have been
a lot of requests to log on to my WIKI from people whom I do not know.

I would like to add some more fields (in the Registration Page) for
Validation Purpose so that I know who is accessing my WIKI.

The fields I wish to include are:
* Company Name
* Company Address
* Company Phone Number

How can I customize the registration page in DOKU WIKI to fit my
requirements. 

Is there any where I can find more reference on this matter? Your help will
be really appreciated in this area.
