Not me, although I am pretty sure the sidebar template was the first template :).
My money is on Carl-Christian Salvesen and the markdown plugin on 2005-05-23, see http://wiki.ioslo.net/dokuwiki/markdown. His only potential competitor is Chris Stoll and the original Folding plugin, http://wiki.splitbrain.org/plugin:folding_sections. The earliest date I can find for that is 2005-06-09. The internet archive (http://www.webarchive.org) "appears" to have a version of the wiki:plugins page from 2005-05-27 with a link to folding plugin. However it links to a page version dated July. There were two other plugins released in May 2005, both by Carl-Christian Salveson, CSV and Graphviz.
