[dokuwiki] Dokuwiki patch - page generation time

This patch provides for a qualitative impression of how fast dokuwiki pages are generated. While $conf['allowdebug'] is set, a comment on page time is added to the end of the default template.

Those commenting on performance may be surprised how fast a normal page view actually is. The cache is an important part of good performance so it also adds a comment to show when a cached file is used. Looking for these comments when $conf['allowdebug'] is on is a quick way to verify that the cache is working correctly.
