[dokuwiki] Clickable index action plugin

I've got a simple action plugin. It overrides 'do=index'. Only
difference to the existing index is that all pages are loaded. You can
collapse and expand any namespace without loading the page again.

I'd love to get feedback, issues...or assuming it is good enough, use
it as an example...

Any reason I can't add it to wiki.splitbrain.org/wiki:action_plugins?
Or is there a better place?
