Hi Micki, Michael,

I am also interested in the following features on the URL that Micki 
provided:

Example 6. Creating several index entries for the same text
Example 8. Specifying a page range in an index entry

Currently I use the technique described in:
http://www.sagehill.net/docbookxsl/IndexCleanup.html
and
http://docbook.sourceforge.net/release/xsl/current/doc/fo/make.index.markup.html
which means you have to run the doclet twice. Once to create the xml index 
and 2nd time to create the final index. Needless to say, it takes twice as 
long even though the output of the index is a lot prettier.
