[dbdoclet] indexing and page-numbered internal references

Hi all!

The increased functionality, image titles and table captions have 
been excellent improvements to dbdoclet (in our case, the html2db 
tool).

Wondering if the following features are on/can make it onto the roadmap:

1. Indexes
Would be great if we could insert and utilize index tags entries from 
html, and have indexed DocBook.

2. Page-numbered internal references
Would be great if anchors within the html can be treated as DocBook 
numbered xrefs.
