ISTR there has been discussion about clipboard tools upsetting things in the past - maybe search the list archives. I don't recall what conclusion we came too. There was a bug in OP which upset one of them which I fixed.
There will also be legitimate things going on, for example if you delete a word in bold, OP is in a "bold pending" state, if you immediately enter plain text it will be in bold. Also there is the matter of copying things from the clipboard in style "A" when style "A" is already defined in the document.
