... and this is precisely what I am trying to do, but using a PDF instead  
of a scanned image (why go to the trouble of scanning when you have a PDF  
of the form?). However, there appears to be a problem on printing, whereby  
the PostScript "image" is larger than the bitmap preview on which the text  
boxes were originally overlaid, and so in the printout the text is no  
longer where it was intended to be.
