I don't "get" what the problem is.

I often fill in forms using Ovation Pro, usually they're printed on paper, so I scan the form into an OP frame (using the TWAIN applet) then overlay small text boxes for the information I want to add. These text frames I set to have 0mm inset, align to centre, transparent background colour, centred text. Having created one I ALT+Shift drag copies to the other fields in the form.

The key thing with this is to set the scale of the picture frame with the scanned form in to 100% so things come out the original size. If people really want their original form using, one can set the picture frame to hide the original form and print out on the original paper form.

Anyway once, I wanted to make a paper form fillable by an audience that did not include OPW users. I found a PDF editor (www.cutepdf.com) loaded the scanned pages into that and then the process was much like OPW, except you save a PDF which can be filled in using the usual PDF viewer.

Loading your PDFs into Acrobat or an editor like the above (there are many) must be an option.

This digression is for general interest, I'd still like to understand the problem. Since if the bitmap preview and the PostScript original are not aligning that is a bug.

One thought I have is that GhostScript renders to a page size, and it may be possible to change the setting for that in the command line you set up for the PDF transloader. In your original posting you quoted that command line and one can see PAPERSIZE=a4 in it.
