That is news to me - which may say a lot about what follows.

Correct.

OP can stand a document have more than one page with the same page 
number. However it won't like having more than one chapter with the same 
number.

There is the 'start on left', 'start on right' setting for each chapter.

I have a dim memory of some important point about blank pages 
(printing?). I can't recall or locate the feature off hand...

No you're right there. It is a long term aim to change this.

I might be inclined to take a text editor to the DDL version i.e. for 
combining chapters.
