I can't remember the details, but I know that attempts to contact Mr 
Breakwell failed. As to the careware implications, the help file says, 
"If you find !MakeRTF useful then please consider making a donation to 
my Scout Group! Cheques (made payable to 2nd New Haw Scout Group)", so 
a donation doesn't seem to be obligatory. Perhaps if the Scout Group 
still exists they wouldn't turn down a donation :-)  I've just done a 
Google, and they are at http://www.2ndnewhawscouts.com/ and oddly 
enough, there is an e-mail address for David Breakwell on the site! I 
don't have the enthusiasm to follow this up, though.

FWIW I did send them some money way back last century, though I never 
found it really useful.
