Exactly what they did -- "ip address fake-made-up-hostname".

I take it from that file that they use #'s as comments.

Uhm, no.. What you're doing here is creating fake DNS-like names.  They're 
not true fake DNS names because those would go in "hosts" and would allow 
you to do things like "ping made-up-hostname," but instead, lmhosts allows 
you to do netbios-related things with these fake names.  I suggest you 
read more documentation on this stuff before trying it...
